Notice: Enrollment is conducting a 2012 Census. All Tåîchô Citizens are ask to fill out the Census Notice Form (PDF).  This form will be used to update your file and help us with the first stage of the family tree program.

Forms:

Tåîchô Citizen Application Form (PDF) - This Form is required for new Citizens.

Change of Address Notice Form (PDF) - This form is required to update our records of your new address.

Change of Name Notice Form (PDF)  - This form is required to update our records of your legal name change.

Transfer Notice Form (PDF) - This form is required as notice for a band transfer or a withdrawal as a Tåîchô Citizen.
